Thank you for your interest in the Kitchener-Waterloo Symphony. Please fill out this Donation
request form in order to be considered for a charitable donation of concert tickets.

The following conditions must apply to all applicants:

• Organization must be a registered charitable organization
• Donation request must be submitted at least 4 weeks prior to the date of the event
  
• Organizations are invited to apply once each 12 month period
